Steyn, 32, ranked as the number one Test bowler in the world, clasped his shoulder on his follow through after two balls of his fourth over in England’s second innings and was taken off for treatment by Proteas captain Hashim Amla.

Steyn came back on to the field and 50 minutes after the initial injury was brought on to bowl but managed only three balls before leaving once more, throwing an object to the floor in clear frustration as he headed to the dressing room.

The right-armer, who has been initially diagnosed with a shoulder strain, missed three Test matches against India last month due to a groin problem.

The loss once again of their main strike bowler would be a significant blow to South Africa, who were bowled out in their first innings by England for 214 - 89 runs behind the visitor's first-innings tally.
